HIV Integration and Health Outcomes Project Evaluation (HIHOPE)
This project was funded through HRSA in 2002, uses an IT system to link staff of infectious disease and primary care clinics serving HIV positive patients with their case managers in the community to share information and coordinate care electronically. HIP is partnering with agencies as far east as Wilmington and as far west as Hickory in this project. For more information contact Sarah Cohen.
